Commercial Driving Assessments

Dear patients,

If you are looking for a commercial driving assessment to be done, please note the following:

• If you are a new patient, we require a comprehensive medical history including blood tests and and and prior information about your health.
• We may require a eye test to be done prior to the appointment. Without this, we may not be able to complete the form.
• Since the form requires us to complete a comprehensive medical, we may require you to do some tests before we can complete the form.
• If we do not have enough medical information on the day, we may not be able to complete the form for you on the day.
• This is not covered under Medicare and will incur a fee of $150. This fee will only be charged if the form has been completed. If you do not have a Medicare card and we are not able to complete the form, there will still be a consult fee charged for the visit.

Please note that due to the nature of the form and the information required, there is always a possibility that we will not be able to complete the form for you on the day, especially if you are not a regular patient.
